Columbus Women's Healthcare is a paperless facility. It is our goal to obtain the most accurate information of each of our patients. All new patients are asked to fill out an on-line form which will ask detailed questions regarding your current and past medical information.

Patients who have established care at our office and are coming in for a new pregancy or who not been to our office within the last two years will also need to update this form. This form can be updated at any time.

A username and password are needed to access this form. One will be provided at the time you schedule your appointment.
