Thank you for choosing Columbus Women’s Health. To provide our patients with the best care possible, please bring the following items and information to your scheduled appointment. If any of these items are not supplied at the time of your visit, you may be rescheduled.

  • Photo ID
  • Insurance card
  • Referral (if needed)
  • Minors (under the age of 19) must have a parent present at their first visit.

We, at Columbus Women’s Health, want to insure that your visit with us always exceeds your expectations. As such, we will work with you to make sure that you are seen in the most timely manner possible, even if that requires being seen the same day. We look forward to working with you to make you an appointment that is as convenient as possible.

When calling for an appointment, please have the following information ready: 

  • Name
  • Date of Birth
  • Address
  • Insurance Information
  • Preferred Pharmacy
  • Please be prepared to share where and what you are being seen for
  • Referrals will be required to supply the same information

Scheduling:
We do require a 24 hour advanced notice if you cannot make it to your scheduled appointment. Please call as soon as possible if you know you will not be able to keep your appointment. If you have young children who will be accompanying you to your appointment, please try to bring another adult with you to sit with the children during your time.

Minors:
Minors (under the age of 19) must have a parent present at their first visit.

Please contact your pharmacy to request a refill prior to calling our office.

Drug prescription refills are filled only during office hours when the physician and nurse have access to your medical record. Please have the phone number of the pharmacy of your choice available at the time of your call.
